Registration Information

All players
attending tryouts must complete a
registration packet that includes:
|

Copy of Birth
Certificate, Copy of USA Hockey IMR,
Player Information Form, Financial
Obligation Form, Consent to Treat
Form, Player Code of Conduct Form,
Parent Code of Conduct Form
|

|
|
| |
Each registration
packet must be accompanied by 2
checks:
|
|
| |
One check is a non refundable tryout
fee of $75
|
|
| |
Second check is a commitment fee of
$200. |
|
| |
If player accepts a position on the
team, the commitment fee will be put
towards the players season dues. If
player does not accept a position,
the fee will be refunded.
